You were directed to remove the tank by April 18, 1989 in the letter sent to you dated October 18, 1988. The tank is located within a commercial area and within a critical zone of contribution to public water supply wells. The potential release of fuel oil, containing carcinogenic and toxic compounds, is a serious threat to the health and safety of the public. SCALE: AS NOTED DATE: MAY 1976 DOWN CAPE ENGINEERING ROUTE 6A YARMOUTH, MASS. 9 Soil exam inat ops conducted at the subject site on May 18, 1976 in the area proposed for subsurface sewage disposal indicate that the natural soil, beneath fill and topsoil, consists of clean sand and gravel which has a percolation rate of less than 2 minutes per inch. Ground water was not encountered to elevation 83.7 feet. The plan proposes to dispose of 4000 gallons per day of sewage from the subject project by means of a 7000 gallon concrete septic tank, a distribution box and four leaching pits with a total available leaching area of 2023 square feet. The Depaq ment of Environmental Quality ig hereby approves the plan with the following provisions: 1. A e�< inch thick reinforced concrete slab mL. . be provided beneath the precast ,,Vtic tAnk in lieu of the twelve inch compacted gravel base shown on the plan. .z. A cast iron frame and cover, at finished grade, must be provided over the distribution box. 3. Each leaching pit must be lowered two feet which results in an invert elevation of 93. 44 feet and a bottom of pit elevation of 85.44 feet. 4. Construction shall be in strict accordance with the approved plan and Title 5 of the .Environmental Code and no further changes will be made in the approved plan without the prior written approval of this Department. 5. A Disposal Works Construction Permit must be obtained from the Barnstable Board of Health prior to the start of any construction. 6. Written certification that the disposal facilities have been constructed in accordance with the approved plan and Title 5 of the Environmental Code must be submitted to the Barnstable Board of Health -with a copy to this office by your :engineering company prior to the system being backfilled. Nothing in this provision is intended to interfere with the right of the Board of Health to inspect the disposal facilities at any time during construction. The Depot Lounge shall not be occupied until a Certificate of Compliance is issued by the Barnstable Board of Health. No environmental assessment form is required to be submitted for this project since it is exempt under the Environmental Protection Regulations of the Executive Office of Environmental Affairs, and the project has therefore been determined to cause no significant damage to the environment.
