SUBMITTAL, NOT APPROVAL OF APPLICATION
Although a file # is being issued, please note the following:
*BRP WM04 License will be required for the aluminum treatment.
*Applicant to review with Waterways program for temporary dock.
*The project site appears to be located within an Estimated Habitat of Rare
Species as designated by the Natural Heritage and Endangered Species Program
(NHESP), and as such a copy of this NOI with site plans must be reviewed by
NHESP per 310 CMR 10.37. Proof of submittal to NHESP missing.
*Conservation Commission, please note that per 310 CMR 10.53(4)(a)3. the
project is to be carried out in accordance with any time of year restrictions or other
conditions recommended by the DFW.
ADDITIONAL REQUIREMENTS:
A Chapter 91 license may be required. Application and transmittal forms are

This project proposes a discharge of dredged or fill material into “Waters of the
United States” potentially subject to the federal Clean Water Act. Per the
Massachusetts General Permit (MAGP) issued under the federal Clean Water
Act, you may be required to notify the US Army Corps of Engineers, New
England District of this project proposal.
