Arrived on site and met with Chief Francis Pulsifer and his staff, including Deputy Request Closed by miorandd Peter Burke. First Student,Inc. is operating the bus company out of this building owned by David Jones System entry on 12/19/2012 9:41:32 AM: of 139 Shallow pond Drive, Centerville, MA.Jim Quinlan was the manager on site for First Student. -Request Closed by miorandd-email sent to Booms and speedi-dry had been placed on the requestor concrete floor.The AGT toppled over allegedly while a worker was filling the tank and somehow the waste oil drain cart was pushed into the tank thereby collapsing the leg on tank and causing it to fall over spilling the waste oil contents. Pictures were taken, printed for file and emailed to Barnstable Fire Dept. Julie Hutcheson of DEP arrived on the scene and issued an NOR. Frank Corp of New Bedford was called in to perform the clean-up.At least 6-55 gallon drums were filled on site with waste. Four workers from Frank Corp were on site. DEP required them to hire an LSP(licensed site professional) In the area of this spill was a sealed old hydraulic lift. Mr.Jones stated it was sealed 20 years ago. Chief Pulsifer stated that a year ago he had told them to put vehicle barriers around the tank. If that had been done this accident could have been avoided. Chief Pulsifer will be issuing fines for failing to perform this requirement.
