Thanks for meeting with me yesterday. The Town of Barnstable Board of Health has determined that using,
storing, generating and/or disposing of 111 gallons or more of hazardous materials requires businesses in the
Town of Barnstable to obtain an annual Hazardous Materials License. The Hazardous Materials ordinance can
be found online at http://wviw.townofbarnstable.us/TownCierk/TownCode.Ddf.
As we discussed, requirements for the license include submittal of the application
(http://www.townofbarnstable.us/HealthDivision/Applications/hazapp.pdf and fee, an inventory of the typical types
and quantities of hazardous materials stored on site, and a spill contingency plan. Please post a copy of the spill
plan on site as well as sending in a copy with the application.

EMERGENCY RESPONSE AND HAZARDOUS MATERIALS CONTINGENCY PLAN
This site-specific Contingency Plan is the facility's plan for dealing with emergencies and must be implemented
immediately whenever there is an imminent or actual fire, explosion, or release of hazardous materials which
could threaten human health and/or the environment. At least one copy of the plan must be maintained at
the facility for use in the event of an emergency and for inspection by the local agency. A copy of the plan
and any revisions will be provided to any contractor, hospital, or agency with whom special (i.e., contractual)
emergency services arrangements have been made.

F. Emergency Procedures: .
Emergency Coordinator Responsibilities:
1. Whenever there is an imminent or actual emergency situation such as a explosion, fire, or release,the emergency coordinator(or
his/her designee)mus:
a. Notify appropriate local authorities(call 911).
b. Activate internal facility alarms or communications systems,where applicable,to notify all facility personnel
b. Assess possible hazards to human health or the environment that may result from the explosion,fire,or release.
c. Identify the character,exact source,amount,and areal extent of any released hazardous materials.
d. Monitor for leaks,pressure build-up,gas generation,or ruptures in valves,pipes,or other equipment shut down in response to
the incident.
e. Take all reasonable measures necessary to ensure that fires, explosions, and releases do not occur, recur, or spread to other
hazardous materials at the facility.
f. Notify the Safety Engineer Leon Lackiram 603-216-8342
2. Before facility operations are resumed in areas of the facility affected by the incident,the emergency coordinator will:
a. Provide for proper storage and disposal of recovered waste, contaminated soil or surface water, or any other material that
results from a explosion,fire,or release at the facility.
b. Ensure that no material that is incompatible with the released material is transferred, stored, or disposed of in areas of the
facility affected by the incident until cleanup procedures are completed.
c. Ensure that all emergency equipment is cleaned,fit for its intended use,and available for use.

G. Post-Incident Reporting/Recording:
The time,date, and details of any hazardous materials incident that requires implementation of this plan shall be noted in the facility's
operating record.
Within 7 days of any hazardous materials emergency incident or threatened hazardous materials emergency incident which triggers
implementation of this plan, a written Emergency Incident Report, including, but not limited to a description of the incident and the
facility's response to the incident,must be submitted to the Safety Engineer, Leon Lackiram. The report shall include:
1. Name,address,and telephone number of the facility;
2. Date,time,and type of incident(e.g.,fire, explosion, etc.);
3. Name and quantity of material(s)involved;
4. The extent of injuries, if any;
5. An assessment of actual or potential hazards to human health or the environment,where this is applicable;
6. Estimated quantity and disposition of recovered material that resulted from the incident;
7. Cause(es)of the incident;
8. Actions taken in response to the incident;
9. Administrative or engineering controls designed to prevent such incidents in the future.

Gentlemen:
In accordance with 310 Dn 15.09(1) of Title 5 of The State &Ivlronmental Code, the
Department of Environmental Quality Engineering has had an engineer review your request
for prior approval to install a sewage ejector at the subject location.
The Department does not recommend pumping into the septic tank, but whereas the
sewage flow being pumped is a small percentage of the total daily flow and should not
cause a major disturbance; the Department hereby approves the proposal with the
following provisions:
1 . The installation meet the requirements of all other State and local agencies.
2. The sewage ejector shall be a low flow (15-25 gallons per minute) , non-
grinding pump.
Please be advised that the installation of a sewage ejector constitutes an
alteration to your subsurface sewage disposal system and therefore, the appropriate
permits for such an alteration must be obtained from the Barnstable Board of Health
in .accordance with 310 CMR 15.02 of Title 5.
