Tank removal was performed by LCR Tank Services of Osterville. We arrived on site at 10:30 a.m. and left the site at 11:15 a.m. During this time we did observe discoloring of the soil and also the odor of #2 fuel oil in the soil at the time. Brett Moscatiello of LCR asked if we had any knowledge of private wells in the area. We indicated that to our knowledge there were no wells present in the vicinity and that the area was seviced by town water. COMMENTS: During the inspection Mr. Magruder had shown us (Health Department and Fire Department) the basement area to look at the foundation wall for any evidence of oil contamination. No contamination was found. The site at this time was left in the hands of LCR for cleanup of the spilled #2 fuel oil. COFFIN, CAPTAIN FIRE PREVENTION FIRE PREVENTION REPORT GENERAL INSPECTION AND FOLLOW UP REPORT INSPECTION DATE: November 22, 1988 INSPECTION TIME: 0945 PROPERTY INSPECTED:241 Indian Trail ADDRESS: PROPERTY OWNER/REPRESENTATIVE AT LOCATION: Lloyd Magruder HAZARDS—VIOLATIONS NOTED: 1. Upon arrival I noted the 275 gallol fuel storage tank. It appeared to have been leaking. When tilted,. the tank was seeping from the end. The dirt in the excavation hole had an odor (strong) of fuel oil. The dirt in the =places where the ends of . the tank rested were also discolored. ' 2: Barnstable Board of Health was notified and Ms. -Donna Miorandi made a site inspection. 3. `' At lOQ7 .hours I notified DEQE in Lakeville and advised. them that a tank �had been leaking on that property. I also, advised them that this tank h, been abandoned. I spoke with Mr. Richard Packard of DEQE. 4.
