During the public meeting of the Board of Health held on January 20, 20034 the Board
reviewed the above referenced preliminary subdivision and makes the following
recommendations:
• Percolation tests must be conducted on each lot at each leaching location, before a
disposal works construction permit will be issued. Maximum groundwater elevations
must be determined by using data available from the United States Geological
Survey- Probable High Groundwater Levels on Cape Cod, Massachusetts and/or by
other methods for determining such elevations'allowed under State Title V and local
Board of Health Regulations.
• Each proposed septic system must conform to 310 CMR 15.000, the State
Environmental Code, Title V, and the Town of Barnstable Rules and Regulations.
• Each septic system shall be located within the prescribed boundaries of each
individual lot.
• Each and every dwelling constructed within this subdivision shall be serviced by
public water.
• Any lawn area created must be covered by at least four(4) inches of loam.
• All tree stumps, brush and building debris removed when clearing lots or roads must
be disposed of at a licensed solid waste disposal facility. Chipping brush and tree
stumps is an acceptable alternative. Burial on site is prohibited.

• The applicant must receive an Order of Conditions from the Conversation
Commission, if applicable.

1. provide access to 10 or more dwelling units; and/or
2. provide access to 4 or more acres of non-residentially zoned land; and/or
3. is a multi-family or non-residential subdivision with access to, or within 500 feet
of Route 132, Route 28 or Route 6A; and/or
4. if the Board deems it appropriate in light of special circumstances, based on

The applicant should request a determination from the Planning Board as to which
sections should be completed, preferably prior to the submission of the Preliminary plan.
Plans should be designed so as to mitigate impacts upon natural resources and
