Statement of Facts: 1. On 8-12-20 an inspection was done at your property and there were several violations that were listed and you were going to take care of ASAP. (see attached ) This sheet spells out the individual violations. Listed below a. SDS sheets not available b. Spill Plan not posted c. Site contingency plan containing all emergency numbers not posted d. Labeling of all hazardous waste e. Waste oil barrel not in secondary confinement f. No spill kit available g. This inspection was conducted in response to a complaint filed with the Public Health Division. The complaint stated that a car radiator was being flushed on the property and that wastewater and antifreeze were flowing down the street. During the inspection,Mr. Lavelle observed puddles of rusty water on the property and on the street. No evidence of antifreeze in the puddles was observed. This was confirmed by the customer who had brought his vehicle in to repair a leaking hose,who said he's continuing to replenish leaked water from the radiator. The following violation of the Town of Barnstable General Ordinance, Chapter 206 Streets and Sidewalks was observed: 4206-3 Impure Waters Rust polluted wastewater was allowed to run off the property into a town street. This water should be collected on site. Once the rust is allowed to settle out,the water can be poured off into the onsite septic system, and the rust discarded in the regular trash. You have been issued a warning notice to cease and desist any illegal dumping-of wastewater into town streets. Any future violations will result in a fine of$100.00 per , violation. Train and practice for effective spill response at least annually. II { a HOW TO RESPOND TO SPILLS 1. Attend to Personal Injuries. Clothing on fire: Roll person on floor to smother flame, drench with water if immediately available. Splash in eves: Immediately rinse eyes with water continuously for 15 minutes. Forcibly hold eye open to rinse behind eyelids. Obtain medical attention. • Spill on body: Remove contaminated clothing and flood exposed area with running water from faucet or safety shower for 5 minutes. Make sure spill has not accumulated in shoes. Minor cuts and puncture wounds: Wash vigorously with soap and water. i Report all personal injuries to your supervisor. P 2. Assess the Risk. • Simple spills meet all these criteria: • Do not spread rapidly • Do not endanger people or the environment except by direct contact • Can be managed safely by people trained to use the material.
